Thank you for your participation in the Healthy Smiles by 2020 Challenge!

The purpose of this Challenge is to increase the number of pediatric patients aged 6-48 months who receive fluoride varnish applications at office visits. Currently around 25% of Maine’s children in this age group are receiving fluoride varnish, the need to improve oral health is substantial. The Healthy Smiles by 2020 Challenge aims to increase fluoride varnish applications to 60% with your help. To help measure the growth with this metric, we ask that your practice self-report baseline data for 1/1/18-12/31/18 from your Electronic Health Record or other record keeping system. Data will be collected bi-annually – mid-year and an end of the year roll up report.
