MaineCare Benefits
The application of topical fluoride is covered for members under the age of 21 with moderate to high caries risk. MaineCare will cover 2 applications per calendar year. For members with high caries rates or new decay within 18 months as documented in member's record, Maine Care will cover up to 3 applications per year.

Employers in Maine Providing Fluoride Varnish Benefit in their Medical Plans
The following employers have agreed to cover fluoride varnish as a medical benefit for children under the age of 42 months. Medical providers can be reimbursed twice per calendar year.
